Week 2 completed and only 2 lbs total loss

Hi, I'm 47 and have a BMI of nearly 32 and 70 lbs to lose. I have tried every diet and healthy eating plan imaginable for years and am very controlled in everything I eat and drink (no alcohol, no coffee, no diet drinks, no refined carbs or sugars of any kind and organic, fresh, vegetarian/fish only, natural products) but I just keep gaining weight. When I was younger I was always very slim (size 6 to 8) without even really trying and then 5 years ago I just started gaining weight despite no particular alteration in my diet and exercise regime at that time and I have now gained over 30kg in total despite all my efforts (I am now much more controlled than I was 5 years ago when this all started!). 30kg is a lot to gain when you are only 5'2". I have checked nothing is wrong with me (I was even referred to a hospital endocrinologist who monitored my blood and urine tests for 2 years and could find nothing wrong. I also had my thyroid and oestrogen rechecked last week and they were normal), so now my GP has suggested I try a VLCD so I started exante 2 weeks ago exactly. I haven't cheated once - I only drink water (that's normal for me) and at least 3 litres a day - it's not my TOTM - I have used the same scales and weighed myself first thing in the morning (naked) after peeing. I go to the gym 3 times a week, swin once a week, attend pilates twice a week and walk everywhere I go. I was not on a low carb diet or any particular diet prior to starting exante - just my regular healthy eating plan. I cannot think what I am doing wrong but I have only lost 2lbs at the end of 2 weeks. I didn't weigh myself the first week as I was away from home at the time. I think I am in ketosis as my breath is bad (although I guess I can't be sure it's like this 100% of the time) and I feel very cold. From what I'm reading this is clearly not normal. Have other people experienced this and does the weight loss eventually kick in? I'm worried that if 2 lbs is all I can lose in the first weeks my weight loss will just stop altogether this coming week. Help!!!
 
Sorry I've no advice but I'm sure somebody will be along... I'm sure there will be a reason.
