what does the cdc do with your forms?

I have a folder of current clients in alphabetical order .. and a folder of ex clients - who I keep as they often come back! We don't disclose them to anyone else though.

We keep the forms ourselves, although they are the property of the Company. The company can at any time request our forms for audit purposes.

If a CDC resigns the forms must be returned to the Company.

If a customer transfers to another counsellor the forms can be transferred over.
