I've always thought it to be, you've lost weight - so a compliment.

yeah, I said it my OH's grandma the other week, she has lost about 2 stone. It's a compliment, but with out having recognised how large the person was before.

yeah my old boss says i look like i put on weight (havent ) but she said " you look well have you gained a bit of weight!! i was like wtf lol.
