Restart question

Hi all, Planning on restarting a week from today, just one question....My BMI is over 40 - will I need to go back to the docs for him to sign me off again?

i was told by cdc ( not a very good one so might be wrong ) that if your forms had been filled in less than 6 months previous then they could use that form if longer thou need a new form

i had to get new forms as mine had been filled in 9 months previous which she didnt fully "get" till i brought her the form as requested and she said they were useless to her
