Struggling with eating patterns and work!

Hi, everyone!
im fairly new to all this lark, so apologies if I'm doing anything wrong!

Im a care worker in the community so work approx 60+ hours a week, and if I'm not driving or working then I'm asleep! I'm really struggling with work and the diet (or should I say lifestyle change!) I take fruit and carrot sticks in the car with me to work etc but when I get home all I want is the most stodgy, pastry covered naughtiness I can fit in my mouth!
Ive maintained the last couple of weeks and am getting a bit fed up because I've done so well so far! I just feel like its all coming to a halt.

if anyone has been in my position or could offer any help I would be extremely greatful!

My hubby works two days two nights and I do slimming world and freeze the rest so when he is doing shift work he takes a pack of the ready made rice and a tub of the meal of his choice and warms it up at work
