SW food diary template

Would anyone happen to have created a SW food diary template that I could copy that matches the one they hand out in class? I find those ridiculous as there is not nearly enough space to fill everything in and it just ends up looking a mess.

I'm sure I could probably create one myself in Word, but if someone has already done so it would save this old git a lot of work! Many thanks.

I find them no good either, I just bought a notebook and write eveything down in there.
