I usually take nurofen. Powerful drug for inflammation and general malaise, and I am pretty sure it doesn't contain anything that can knock you out of ketosis...

Though I have read that infections themselves can do that, as the body turns to burning lean tissue rather than fat during infections.

Is there any general guidance about what to do during illness on ketotic diets?

If it does, it must be minuscule and hardly enough to provide enough carbohydrate to excuse the body from burning its own reserves. That coating is meant to be "enteric" so that it waits till it gets into the bowel to be dissolved instead of irritating the stomach. As the stomach can digest sugar, I would imagine the coating is not intrinsically sugar.
