I'm going to post this on the pregnancy board too but thought I'd ask here.

My midwife has said I don't get this form till I'm 26 weeks pregnant but my employer says they need to have received it before I'm 25 weeks pregnant so which is it?

I can't remember when I got it with my last baby.

    Can anyone help?

I don't know for sure but I reckon your midwife is wrong as yes, you do have to give your employer notice of going on maternity leave and some people choose to go earlier than others. Most people I know got the form at their first midwife appt.

    I think that's a requirement of being a midwife sometimes ;)
